Why Invisalign?

When was the first time you thought about improving your smile? Was there something that put you off? Did the thought of cutting back your natural teeth..all that ‘drilling’ put you off? or was it the thought of having big metal brackets beaming out of your mouth every time you smiled that made you think twice? Invisalign is an american branded product which allows adults (and teenagers) to have there smiles transformed, WITHOUT the need for traditional braces, or destructive drilling. It is a virtually clear, removable appliance which has revolutionalised the dental industry!

How does it work?

The first thing out of nearly EVERYONES mouth when they see how thin the aligners are is, ‘how can something so small move my teeth?’. It’s simple, which ever orthodontic system you use, teeth are moved by putting pressure on them. With traditional braces this is done by increasing the tension in the metal wires, if you’ve ever heard of, or experienced having your ‘wires tightened’ you’ll know what I mean! Invisalign works by having a whole series of aligners, each one is in a slightly more moved position than the previous one, and that’s how, every two weeks when the patient changes over their aligners, it puts gentle pressure on specific teeth to move them into their new position!

How often do I need to come in for visits?

We always like to keep close contact with all our Invisalign patients, especially at the early stages where patients are just getting started with their Invisalign treatment, however once you start, unless there is a specific reason, most patients will visit the practice every 6 weeks or so. You’ll appointments will be with one of our certified Invisalign practitioners.

Your childs first visit

A childs first visits to the dentist are very important, perhaps the most important visits of their life as it can set the tone for how they view dental treatment for the rest of their lives! Will they become phobic or scared of the dentist so that they begin to avoid going and neglect their teeth leading to problems as an adult, or will they be regular attenders who enjoy their routine dental care visits, look after their teeth and have much fewer dental problems and treatments as they grow up?! This is why our first introduction visit is so important!

• Tell, show do - we’ll intoduce your child to the dental surgery – it can be an unsual and intimidating enviroment for children, especially on their first visit, we’ll take our time to make sure they feel at ease and are comfortable to sit on the chair. We use ‘show, tell, do’ approach with our children patients (as well as our adult ones infact!), here we’ll tell them what we are going to do, we’ll the show them the instruments (dental mirror) we are going to use and make sure they are happy for us to use them to look at their teeth, and then we’ll do it.

• Home care advice - we’ll also dicsuss with you the best and most appropriate way to care for your childrens teeth.

• How many teeth do you have?! - we’ll check that your childs teeth are developing as they should be.

Antonis graduated from University of Athens, Greece, in 2003 and has been practising in the UK since then. His mainexpertise  is Implant Dentistry, for which he holds a Master in Science from the University of Warwick in Implant Dentistry.

He is also an Honorary member of staff at Warwick, guiding, supervising and training dentists. Antonis has placed and restored hundreds of implants. He has attended both national and international courses relating to both general and Implant Dentistry, he is also a certified Invisalign dentist.

In this series of blog posts we will be looking at the best ways of whitening your teeth, the different ways you can whiten your teeth, insturctions for how to whiten your teeth, what your other treatment options are as well as looking at more complicated teeth whitening cases.

There are 2 main ways to whiten your teeth. Either using a home whitening kit (professional kit supplied by your dentist or any of the many products found online or elsewhere!) or an in’surgery whitening also known as ‘power whitening’ or ‘laser whitening’. Now, one thing I tell ALL my patients, if you want the BEST results, QUICKEST, you should do a combination of the home and In-surgery whitening.

Home Teeth Whitening

The method that we (and most dentists) use for home whitening, is using ‘bleaching trays’  (see image 1). These are thin pastic trays which are made by taking impressions of your teeth, from this we can make your bleaching trays so that they are customised to you and very close fitting to the gums and teeth to prevent any gels from escaping! This is a big difference between trays your dentist will make, and those DIY trays you can buy over the interenet, these trays will be ‘best fit’ and will often have gaps and voids in them, allowing gels to escape, which not only makes the whitening not as affective, but also means gel (basically a bleach) may leak into the mouth, or even be swallowed, which can become dangerous!!

Patients using a home kit on its own, on average will use the kit every day/night consecutively (if possible) for around 2-3 weeks, unless combining this with in surgery in which case only 4-5 days of the home kit is normally needed (see cobination whtiening section below).

We provide our patients with an options of gels to use, either our day gels which breakdown quicker and can be used during the day (for 30mins- 1hr), or night gels which break down more gradually and can be used over night (4-6hours). With both of these options our patients also have a choice of higher or lower concentration depending on weather we think they may have sensitivity.

One of the great things with having the home kit, is that you will be able to keep your new smile white for life! Patients can simply ‘top-up’ whenever needed in the future, your trays should fit you for life (unless your teeth move), and gels can be purcahsed seperately from our reception.

In- Surgery Teeth whitening

One of the most pooular ways of whitening your teeth now days is in-surgery whitening, also know as ‘power’, ‘laser’ or ‘in-office’ whitening. We do hundreds of in-surgery whitenings and have been doind so for many years, finding it a safe, simple and effective way of whitening teeth.

In-surgery whitening, works in the same manner as the home whitening in that we use a bleaching gel to whiten the teeth (this is a much higher concnetration that those given to patietns for home use), as well as using an ‘accelerator’, this is normally in the form of either a light/laser/heatsource. Much of the marketing and promotion around these different accelerators are advertising gimiks, basically, they all give off heat which speeds up the reaction (basic chemistry 101)!

The treatment is quite straight forward, patients will come in, after explaining the procedure to them, we will spend the first 10-15 minutes ‘isolating the soft tissues’ (see image 2) this means protecting all the gums, cheeks, tongue (and anything else not teeth!), once all this is in place, the bleach and the accelorator is applied and left for between 15-30 minutes (depending on the case), this is then removed and reapplied 2-3 times during your session. At the end of the treatment all the gels and products are removed and thats it !

As the whole treatment can take an hour or so, we provide many options for patient entertainment, from wireless headphones with hypnosis relaxation tracks, dvd glasses, tv or dvd’s to watch or a selection of magazines. Patients are also welcome to bring their own laptop to work on, or ipods/mp3 if they would prefer to listen to their own tracks. Image 3 shows one of my patients having their teeth whitening whilst watching a film using our wireless headphones!!

5 Tips to get the best results from teeth whitening

1. Use a combined option of home & In-surgery, not only will this give you the best and quickest results, it will also allow you to maintain this colour for life!
2. Have your teeth professionally cleaned by a hygienist or dentist before your whitening. Even with the best whitening in the world, visible plaque and tartar will spoilyour new white smile, your dentist/hygienist will be able to remove this for you, having this done before teeth whitening will allow the underlying tooth to be whitened as well.
3. Be smart about sensitivity! – Sensitivity is a normal side affect of teeth whitening, in 90%of cases it is very mild and requires little or no management, it is always temporary and usually fades within 2 weeks of stopping the whitening. If you are concenred about sensitivity or know you have sensitive teeth, you should discuss this with your dentist before hand and they should be able to provide you solutions. WY10 the whitening system we use, has the lowest sensitivity we have ever found, in addition to this, we provide our patients desensitizing toothpastes and gels if needed.
4. Follow up – make sure to follow the schedule recommended by your dentist, often, even if we are really happy with the result following an in-surgery session, we will recommend the patient to follow up with a couple of days with the home kit. This is to help minimise any ‘fade back’ where the teeth get less white the week following an in surgery whitening as they rehydrate.
5. Don’t be a stranger !! The best way to keep your stunning new smile, as well as using your home kit, is to visit your dentist/hygienist regularrly for professional cleaning. This will help to remove surface staining before it has the opportunity to penetrate deeper into the tooth making it appearing yellow

One of the most enjoyable parts of the facial aesthetics that we offer at our practice, is working with dermal fillers. We use brands such as Restylane and Juvederm, which are non-permanent dermal fillers, they both are based on molecules which are naturally present in the skin and are naturally broken down in the body.

This is a really great and simple case from one of my patients some time ago. The challenge here was to make the lips fuller, without losing their natural great shape or looking like they have been artificially created.

Here we used one full syringe of Juvederm Ultra, which is a great product not only because it is so smooth but also because it has anesthetic in it, it makes the whole process even more comfortable and enjoyable for the paitent.  What patients love so much about dermal fillers, is, not just the great results that they produce, but also that the results are immediate!

As you can see from the photos here, we’ve produced a really great result, enhancing the natural beauty of the lips, without looking ‘fake’!

Of all the area of the face where fine lines can appear, those surrounding the corners of the eyes are one of the areas that patients tend to dislike the most. Although sometimes present at rest, they are most noticible when smiling, hence their name ‘smile lines’.

One of the most effective ways to treat these areas is with botox. Botox is a neurotoxin which prevents the muscles from working. This relaxes the muscles and in turn reduces the appearance of these fine lines.

Many patients do not want to look like they have had any treatment done, and whilst they would like these lines reduced, they also want a natural look. I think this is very important, and that’s exactly what we aim to provide for our patients. As you can see from this case, although we’ve soften these lines really well, we’ve also left a few very faint lines towards the lower half of the eye, this gives a much more natural appearance as well as allowing the patient to smile normally!

This is case is really exciting because it is one where traditionally we would have either used orthodontics (braces to move the teeth), which would have taken a considerable amount of time probably a minimum of 1 yr, or we would have used crowns/veneers, which would have involved a considerable amount of cutting back of the natural, healthy teeth. Instead we decided to do composite bonding.

Composite bonding is using a white filling materials (composites) to change the size, shape or colour of teeth. Natural teeth aren’t all one flat colour, they have a variation of different shades, tints, colours and translucency throughout an individual tooth, this is what makes them look natural!! We carefully select a variation of different colour composites and blend them together to recreate the tooth’s natural colour and translucencies, this is one of the ways we get such natural looking results.

This amazing transformation was undertaken in one visit and was a really great case that all our team were excited about being a part of.

A lovely result for our patient, and a great experience for all our team.

Case 2 (composite veeners) – image missing to get from laptop

As you can see from the before and after photos here, this patient had previously had some white fillings done on the front teeth, bit by bit over the years. Some wear and tear along with a staining and some tooth wear had given these anterior teeth an appearance that our patient wasn’t satisfied with and wanted to improve, without having to cut back lots of natural healthy teeth. So we decided to simply replace these old composites and replace them with composite veneers (LINK), this is exactly the same as composite bonding except that the composite covers the whole surface of the tooth, just as in a porcelain veneer. We also did some teeth whitening here as well just to lift the shade and give an extra something to the final smile.

These are 2 great cases we’ve recently completed using a combination of teeth whitening, Invisalign and cosmetic bonding to create the perfect smile. Our patients have been kind enough to share their pictures and journey with us so that we can show you just how we can transform a smile J.

Case 1 (DC)

This patient was unhappy with the gaps around all the anterior teeth. We started this case with Invisalign, and as you can see from the Invisalign digital treatment plan, there was a really great improvement in closing the gaps. Once we finished the Invisalign treatment we decided to do a small amount of cosmetic bonding just to completely close any remaining gaps and slightly change the shape of the teeth for a more pleasing aesthetic result! As you can see a truly amazing result!

Case 2 (AN)

This patient has what is called ‘peg shaped lateral’, this is quite common, in fact these 2 teeth (the lateral incisors) either side of the two front teeth are the most commonly either missing or misshaped teeth.

We decided with the patient to use Invisalign in order to create more space so that we could build up the teeth to a better size, shape and proportion. Here we used a combination of in-surgery and home whitening to get the best results fastest .

Invisaligns digital treatment plan is great for planning cases like this as we can be very specific with exactly how much room we want to create around each tooth. From here its just a case of getting the right colour and creating the perfect smile.

The really nice thing about both these cases is that with composite bonding we’ve been really conservative with the teeth and haven’t had to cut back that precious tooth tissue, because of this, no aesthetic is need at all and the patient can have their new smile in just one visit !!!
